LG Wireless Phone

LOCK CODE problems on a verizon wireless LG phone.. HELP?!?!?!?!?!?

i have the verizon wireless LG T phone (it is my old one) and i want to use it now.. and i locked a long time ago and cannot remember the code.. does anyone know of a way I can unlock it.. HELP PLEASE!!?!?!?!?!?!?!!! i changed it to a different code besides the last four digits or 0000 and i need to unlock it!!
