Verizon Wireless Insurance ?

Okay well i am going to call later on today because my phone cracked and i was wondering what do they ask fro ? do they ask for the social security number the account number or what? please help here.

1. Either the account number or the mobile telephone number and the password or the last four of the account owner's SSN. If you are within the first 30 days of owning the phone the verizon will replace the phone under the warranty for free. If past the thirty days and you have insurance then you will be referred to Assurion to file a claim with a $50 copay. If it is just a cracked screen and you are past the thirty day warranty and under one year, verizon will replace it for a $50 fee.
